    Assessment Description

You are a counselor working at a publicly funded alcohol and drug treatment program. Your client, Doug, attends one of your counseling groups. He has a previous conviction and is on probation for possession of an illegal substance. He also entered treatment as a court-referred client. Doug's probation officer is aware that Doug is in treatment, and the officer has sent you a letter requesting that you provide a progress report and an assessment of Doug's likelihood of relapse. The probation officer will use this information, if favorable, to petition the court for Doug's early release from probation.

Discuss your options and strategies for responding to the probation officer's request and describe what you would do. Cite relevant legal issues, including federal confidentiality and privacy regulations in your discussion. Identify the specific circumstances when you would provide the information requested by the probation officer. How would you handle this differently if Doug were voluntarily attending the group?

In your response, you must include the specific code(s) from the ACA Code of Ethics or the ASCA Ethical Standards for School Counselors that address the concerns raised in the case. Be sure to provide the full text of the code followed by an in-text citation and a full reference at the end of your response.

To strengthen your response, you may also include citations and references from the Corey text or other assigned readings relevant to the topic.

Identify a health care need that is currently a concern in your community or state( MIAMI, FLORIDA). For example, perhaps you find that your state has one of the highest rates of heart disease in the nation. Once you have identified the health care need in your community, do further exploration to see whether certain groups or populations within your diverse community or state are impacted by this health care need more than others. In our example, elderly and Native American populations are two groups that have a higher prevalence of heart disease.

Appropriately identifying the highly impacted subgroups within your community or state is a key component of this assessment, as you will continue to reference these groups for the upcoming assessments as well.

For this assessment you will develop a concise but comprehensive report of the demographic makeup of the community you have decided to focus upon, as well as the most significant health concerns. Be sure you are linking the health concerns you are identifying to specific demographic groups within your chosen community. ·
    **Information dissemination.**

    · This includes using a variety of channels to reach multiple audiences. These activities include general media relations, working with designated spokespersons, organizing news conferences, and providing briefings and updates. These information dissemination activities should extend to web support and social media.

    ·
    **Operation support.**

    · Provide support for your position.

    · Include in your plan contact tracing of the homeless, disabled, displaced community members, migrant workers, and those who have hearing impairment or English as a second language in the event of severe tornadoes.

    **Case Study: Healing and Autonomy**

Mike and Joanne are the parents of James and Samuel, identical twins born 8 years ago. James is currently suffering from acute glomerulonephritis, kidney failure. James was originally brought into the hospital for complications associated with a strep throat infection. The spread of the A streptococcus infection led to the subsequent kidney failure. James’s condition was acute enough to warrant immediate treatment. Usually cases of acute glomerulonephritis caused by strep infection tend to improve on their own or with an antibiotic. However, James also had elevated blood pressure and enough fluid buildup that required temporary dialysis to relieve.

The attending physician suggested immediate dialysis. After some time of discussion with Joanne, Mike informs the physician that they are going to forego the dialysis and place their faith in God. Mike and Joanne had been moved by a sermon their pastor had given a week ago and also had witnessed a close friend regain mobility when she was prayed over at a healing service after a serious stroke. They thought it more prudent to take James immediately to a faith healing service instead of putting James through multiple rounds of dialysis. Yet, Mike and Joanne agreed to return to the hospital after the faith healing services later in the week, and in hopes that James would be healed by then.

Two days later the family returned and was forced to place James on dialysis, as his condition had deteriorated. Mike felt perplexed and tormented by his decision to not treat James earlier. Had he not enough faith? Was God punishing him or James? To make matters worse, James's kidneys had deteriorated such that his dialysis was now not a temporary matter and was in need of a kidney transplant. Crushed and desperate, Mike and Joanne immediately offered to donate one of their own kidneys to James, but they were not compatible donors. Over the next few weeks, amidst daily rounds of dialysis, some of their close friends and church members also offered to donate a kidney to James. However, none of them were tissue matches.

James’s nephrologist called to schedule a private appointment with Mike and Joanne. James was stable, given the regular dialysis, but would require a kidney transplant within the year. Given the desperate situation, the nephrologist informed Mike and Joanne of a donor that was an ideal tissue match, but as of yet had not been considered—James’s brother Samuel.

Mike vacillates and struggles to decide whether he should have his other son Samuel lose a kidney or perhaps wait for God to do a miracle this time around. Perhaps this is where the real testing of his faith will come in? Mike reasons, “This time around it is a matter of life and death. What could require greater faith than that?”

    ### **Scenario**

Health resources are finite. Therefore, it is incumbent on all health organizations to exercise responsible fiscal decision making when allocating their financial resources.

As the senior cost analyst for a local, nonprofit hospital, you are charged with determining the most appropriate use of financial resources and making recommendations. Your organization is seeking to secure a new CT Scan unit for the expanded emergency department. The hospital has the option of leasing the equipment or purchasing the equipment.

The cost to purchase the CT scan is $1,300,000 at 10% (PV), with straight line depreciation over 5 years. The trade-in value $130,000 at the end of its useful life. The maintenance expense equals $12,000 annually.

The cost to lease the equipment is $26,000 per month for a period of 60 months, which includes all maintenance costs. The tables below provide the financial overview of the purchase and lease costs.

    ### **Scenario**

    W. Edwards Deming, who many consider the father of quality management, espoused the importance of using data for decision making. As a healthcare leader responsible for managing your healthcare organization’s population health management program, you will find that relevant, accurate, and current population health data is an essential part of your management toolkit. The exploration of data sets not only deepens your understanding of population health but also expands your awareness of the role of data in both the healthcare delivery system and in the way healthcare leaders can prioritize community needs.

Data sets from federal and state resources, as well as from private foundations and academic medical centers are critical components for promoting evidence-based population health management programs. For this assessment, you will explore and assess the impact of data and information on population health programs and initiatives.
