Create a clinical case using the ICF as a basis. It needs to involve a client and include your chosen health condition and 3 environmental/contextual/personal issues.The health conditions will be based on the content from PHS406.
Utilize the ICF to consider it’s components and other contributing factors to analyse and explain the clients presenting situation.
Demonstrate clinical reasoning to design the client management plan for your initial consultation and then 2 weeks later (eg day 1 and day15) that is supported by evidence-based practice and scholarly literature
Discuss any professional concerns that may have arisen with the case and how they can be addressed
Your case presentation must be creative, interesting and innovative
Information from content will help guide you and you may also use textbooks, peer-reviewed journals, libraries, and the internet to find information to support your management plans
All ideas must be referenced to justify your clinical decisions
You will need to submit case, demonstration of clinical reasoning and reference list links to any videos you may choose to use within the case study
