    Healthcare Organization Services Pricing

In a truly competitive marketplace, an organization must assess what its competitors are charging for services that organization offers. If it charges too much, other organizations simply will not conduct business with it. If it charges too little, the profit margins are non-existent and the organization would fail to make a profit.

HR STRATEGY STATEMENT Worksheet (RLB Group 2009)

An HR strategy gives focus and direction to the HR department. There are many versions and dimensions of strategy statements in general and HR in particular. We suggest that in transforming the strategy of a specific HR department, the statement of strategy (sometimes called strategic direction, intent, or architecture) should answer three questions, in this order:

Who are you? (vision) 2. What do you deliver? (mission or value propostion) 3. Why do you do it? (the results HR wants to achieve)

Who are you? Who we are represents an identity or image, which is what an HR strategy should offer those outside as well as inside the function (ie. thought leaders, change architects, facilitators, or coaches). We have found a simple

question helps frame the definition of identity: What are the top three things we (in HR) want to be known for by those who use our service?

What do you deliver? What we deliver is the set of outcomes described in Chapter 3 of “HR Transformations.”  Defining HR deliverables comes from doing organization audits as described in Chapter 3.

Why do you do it? This question links back to the business context (Chapter 2) and specific business results tied to key stakeholders. These “so that” statements should imply measures that allow you to monitor the progress HR makes and the impact it has on the business.

Directions:  Brainstorm individually, then refine with the entire HR transformation team. Putting the three statements together creates your HR Strategy Statement (you will likely select more than one result for each question but should have enough focus to select no more than three). Fitz-enz’s 11 guiding principles for human capital metrics (HCM) and Reyes HR department

My proposal is based on the care of late preterm infants, and it is a journal that has advances in the care of pediatrics, and the author believes this would be the best organization to bring the proposal to.

Next, the author would present the proposal to a conference of multidisciplinary members of the hospital  specifically one that is centered around neonates because they all would have part in the dissemination of the project from lab,, neonatologist, pharmacy, radiology, occupational therapy, administration, managers and neonate nurses.  Having a multidisciplinary approach would disseminate the information much easier and everyone is on the same page with treatment. Dissemination of Evidence-Based Practice (EBP) Essay Paper – Agency for Healthcare and Research Quality (AHRQ)

  • Dissemination of research findings in the clinical nursing literature

    Give a positive comment on the following dissemination of research findings in the clinical nursing literature post;

Dissemination of research findings in the clinical nursing literature occurred at two levels: through articles that reported studies of potential value to the nurse’s practice and citations to research publications within articles. Relevance to clinical practice. Disseminating research in journals that are geared to clinicians is essential to increase nurses’ awareness of research findings that might be relevant to their practice (Oermann, Nordstrom, Wilmes, Denison, Webb, Featherston, & Kowalewski, 2008).

The American Association of Critical Care Nurses Journal would be a great place to present my project because the success of ventilator associated pneumonia (VAP) bundles in preventing ventilator associated pneumonia largely depends on the knowledge and cooperation of critical care nurses. Educating them will bring awareness to the difference that they as nurses can make. Over 100,000 nurses and nursing students currently subscribe to The American Association of Critical Care Nurses Journal. The National Teaching Institute & Critical Care Exposition would be a good conference to discuss my project because the conference focuses on evidence based practices and has critical care nurses from around the country and beyond. It is a good way to educate but also a great place to learn from others in the same field. The exposition also allows nurses to collect CE hours for attendance so this will add an extra incentive for nurses to participate. The conference offer more than education; it also offers an environment for support and inspiration. Nurses come with an open mind ready to learn and getting nurses to understand the need for change is half the battle. Dissemination of research findings in the clinical nursing literature

Give a positive comment on the following Early Mobility of ICU Patients EBP Project Information Dissemination.

Dissemination of EBP is an important part of practice change. Writing a journal article or presenting at a conference allows information to reach a number of nurses outside one’s clinical practice site. For my EBP project of early mobility of ICU patients, a journal that would be appropriate to help disseminate information is the American Journal of Critical Care (AJCC). This is a bimonthly journal from the American Association of Critical-Care Nurses (AACN) that focuses on evidence-based practice and offers the latest advancements in clinical research (American Association of Critical-Care Nurses, n.d.). This journal goes out to every AACN member, of which there are currently over 100,000. Presenting information at NTI would allow me to share my research and experiences with other critical care nurses from around the country. This year the conference is in Boston and they expect over 7,000 nurses to attend (AACN, n.d.). I have not attended NTI, but I hear it is full of useful education and activities that improve patient care. Early Mobility of ICU Patients EBP Project Information Dissemination

    NRS 490 Week 3 Discussion 1

Submit a summary of six of your articles on the discussion board. Discuss one strength and one weakness to each of these six articles on why they may or may not provide sufficient evidence for your practice change.

    NRS 490 Week 3 Discussion 2

Name two difference methods for evaluating evidence. Compare and contrast these two methods.

    NRS 490 Week 4 Discussion 1

With your mentor, discuss why understanding the health care system at the local level plays an important part in the logistics of an EBP implementation. Discuss one of your learnings from this conversation that you will take into consideration for your own change project.

    NRS 490 Week 4 Discussion 2

Identify two change theories, and in your selection process include asking your mentor which one(s) he or she uses or has used. Compare and contrast each one and then note which theory makes the most sense for implementing your specific EBP project?

    NRS 490  Week 5 Discussion 1

Stakeholder support is necessary for a successful change proposal project implementation. Name one internal (facility, unit or health care setting where the change process is situated) and one external stakeholder (individual or group outside the health care setting) and (1) why their support is necessary to the success of your project, and (2) how you will go about securing that support.

    NRS 490  Week 5 Discussion 2

Technology is integral to successful implementation in many projects, through either support or integration or both. Name at least one technology that you might or will need to work with that will improve the implementation process and the outcomes of your EBP project.

    NRS 490  Week 6 Discussion 1

After discussion with your mentor, name one financial, one quality, and one clinical aspect that needs to be taken into account for developing the evidence-based practice project. For each aspect, explain how your proposal will directly and indirectly impact each of the aspects.

    NRS 490  Week 6 Discussion 2

Access and review the “Capstone Project Topic Selection, Rationale, and Approval” assignment from Topic 1. Now that you have completed a series of assignments that have led you into the active project planning and development stage for your project, take your discussion a step further and briefly describe your proposed solution to address the problem, issue, suggestion, initiative, or educational need. How has this changed since you first envisioned it, and what led to your current perspective and direction?

    NRS 490  Week 7 Discussion 1

Determine one internal and one external method for dissemination of your EBP project. For example, an internal method may be the hospital board, and an external method may be a professional nursing organization. Discuss why it is important to report your results to both of these groups. How might your tactics change for each type of group?

    NRS 490  Week 7 Discussion 2

While EBP is not research, it is important to be able to determine the effectiveness of your change. Discuss one way you will be able to evaluate whether your project made a difference in practice.

    NRS 490  Week 8 Discussion 1

Based on how you will evaluate your EBP project, which independent and dependent variables do you need to collect? What basic statistical analysis might be run to determine statistical significance?

    NRS 490  Week 8 Discussion 2

Not all EBP projects result in statistically significant results. Explain what clinical significance is, the difference between clinical and statistical significance, and how you might use clinical significance to support positive outcomes in your project.

    NRS 490  Week 9 Discussion 1

Discuss one personal strength and one weakness you have regarding professional presentations. Name one method for improvement for each of these, and discuss why it is important for you to work on these skills.

    NRS 490  Week 9 Discussion 2

Sustaining change can be difficult, as there are many variables that can affect the process and implementation. One critical component of EBP is to ensure that practice change is hardwired so that it will thus continue to impact outcomes over time. Name two potential barriers that may impact your EBP change proposal from continuing to obtain the same desired results six months to a year from now, and your strategies for overcoming these barriers.

    NRS 490  Week 10 Discussion 1

Dissemination of EBP and research, such as presenting results at a conference or writing an article for a journal, is an important part of professional practice. Name one professional journal and one nursing or health care conference where you might present our project as a presentation or article and discuss why each of your choices is the best option for you to disseminate your new knowledge.

    NRS 490  Week 10 Discussion 2

Discuss why EBP is an essential component of the practice of a BSN-prepared RN. Then name two ways in which you will continue to integrate evidence into your practice and encourage it within your work environment. What problems or impediments do you foresee in being able to follow your EBP action plan? What steps can you take to minimize these?

    NRS 490  Week 2 Assignment

In collaboration with your approved course mentor, you will choose a specific evidence-based practice proposal topic for the Capstone Project. Consider the clinical environment in which you are currently working or have recently worked. The Capstone Project topic can be a clinical practice problem, an organizational issue, a quality improvement suggestion, a leadership initiative, or an educational need appropriate to your area of interest as well as your practice immersion (practicum) setting. A combination of 100 “supervised clinical hours” in community health and leadership areas will be obtained through the application of the objectives listed in the “Guidelines for Undergraduate Field Experiences” manual and as captured in the ISP.

Practicum immersion experiences are required in a community health setting. Community health and leadership practice immersion can occur in the same site and in conjunction with the evidence-based project in the NRS-490V course. Definitions of community-based settings should encourage community integration and involvement; expand accessibility of services and supports; promote personal preference, strengths, dignity; and empower people to participate in the economic mainstream. Examples of the integration of community health, leadership, and an EBP can be found at Healthy People 2020.

The goals of the practice immersion experience is to apply the learning from your research, community health, and leadership courses:

After studying this course, you should be able to:

Demonstrate and apply knowledge of factors to consider when planning to work with a chosen community.

Demonstrate and apply knowledge of approaches that will facilitate engagement of the community.

Demonstrate and apply knowledge and skills required to work constructively with communities.

Demonstrate and apply knowledge related to engagement of organizational leadership and change agents.

Demonstrate and apply knowledge related to disseminating project findings/data with organizational leadership and change agents.

Demonstrate and apply knowledge related to completing an evidence-based project to impact population health outcomes.

Demonstrate and apply Healthy People 2010/2020 principles related to completing an evidence-based project to impact population health outcomes.

According to Healthy People.gov, educational and community-based programs and strategies are designed to reach people outside of traditional health care settings. These settings may include:

    Schools

    Worksites

    Health care facilities

    Communities

Each setting provides opportunities to reach people using existing social structures. This maximizes impact and reduces the time and resources necessary for program development.

In a composition of no more than 750 words, describe the nature of your proposed project topic. Include the following in your discussion:

The setting or context in which the problem, issue, suggestion, initiative, or educational need can be observed.

A description providing a high level of detail regarding the problem, issue, suggestion, initiative, or educational need.

Impact of the problem, issue, suggestion, initiative, or educational need on the work environment, the quality of care provided by staff, and patient outcomes.

Significance of the problem, issue, suggestion, initiative, or educational need and its implications to nursing.

Preview and utilize the “Project Topic Checklist” resource located in the topic materials to assist in developing your proposed Capstone Project topic. This resource will assist you in organizing your work and will provide additional information regarding the assignment. Note: You are required to retrieve and assess a minimum of 15 peer-reviewed articles. Plan your time to complete this assignment accordingly.

Attach the completed “Project Topic Checklist” to your assignment submission.

Review the PICO(T) resources listed in the topic readings.

Formulate a PICOT statement using the PICOT format used in the assigned readings. The PICOT statement will provide a framework for your Capstone Project.

In a paper of 500-750 words, clearly identify the clinical problem (from your Topic 1 approved Change Proposal) and how it can result in a positive patient outcome.

Make sure to address the following on the PICOT statement:

    Evidence-Based Solution

    Nursing Intervention

    Patient Care

    Health Care Agency

    Nursing Practice

Recall that a PICOT starts with a designated patient population in a particular clinical area and identifies clinical problems or issues that arise from clinical care. The intervention should be an independent, specified nursing change intervention. The intervention cannot require a provider prescription or equipment change. Include a comparison to a patient population not currently receiving the intervention, and specify the timeframe needed to implement the change process.

While APA format is not required for the body of this assignment, solid academic writing is expected, and in-text citations and references should be presented using APA documentation guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

In nursing practice, accurate identification and usage of research is essential to achieving successful outcomes. Being able to articulate the information and successfully summarize relevant peer-reviewed articles in a scholarly fashion helps to support the student’s ability and confidence to further develop and synthesize the progressively more complex assignments that constitute the components of the course change proposal Capstone Project.

For this assignment, the student will provide a synopsis of 15 peer-reviewed articles from nursing journals using an evaluation table that determines the level and strength of evidence for each of the 15 articles. The articles should be current within the last 5 years and closely relate to the PICOT statement you developed earlier in this topic. Appropriate resources to include involve quantitative research, descriptive analyses, longitudinal studies, or meta-analysis articles. A systematic review may be used to provide background information for the purpose or problem identified in the proposed Capstone Project.

While APA format is not required for the body of this assignment, solid academic writing is expected, and in-text citations and references should be presented using APA documentation guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

In this assignment students will develop a 750-1,000-word literature review in which to synthesize what is known and unknown about the Project issue selected. Students will use the information from the PICOT Statement and Evaluation Table assignments prior in the course to develop a planned approach to implementing the evidence-based practice solution. The underlying purpose of this assignment is to analyze how the current research supports the PICOT, and to identify what is known and what is not known in the evidence, providing a logical, constructive argument.

    Include the following sections:

    Title page

    Introduction section

    A comparison of research questions

    A comparison of sample populations

A comparison of the limitations of the study

A conclusion section, incorporating recommendations for further research

Setting up compelling control measures could essentially control the quantity of passings around the world. To control and administration hypertension successfully, it is imperative to recognize and deal with the way of life hazard factors related with the condition as that brings down circulatory strain. What’s more, it is crucial to perform standard screening, discussing adequately with patients, checking patients all the time, and holding fast to treatment. Specifically, it is conceivable to avert and control the condition viably through way of life changes (slim down and frequently captivating in work out) as these serves to altogether direct pulse (Dua et al,2014). Evidenced based solution for patients with hypertension is to lower DBP and SBP below levels in which medication will be started. There should be an alternative drug from a different class to take care of a patient.

    PICOT Question

For male grown-ups between the ages of 40 and 70 with hypertension, and with various co-morbidities, will the adjustment in way of life (drawing in routinely in exercise and eating more beneficial and adjusted suppers), contrasted with patients who utilize prescription to treat/deal with their hypertension, help to control their pulse and diminish the danger of creating cardiovascular illnesses in their recuperation period inside a half year. The period will be sufficiently long to make a patient to be capable not to experience the ill effects of hypertension and to likewise lessen the dangers that the people will ordinarily go through(Howes,2013). Scholarly Activity Summary Template & Example Assignment NRS-490.

    Intervention

The primary technique for intercession for a patient with hypertension it is with no uncertainty to put them under prescription, so they can get cured. That is the most secure path as it will influence the patient to have the capacity to deal with themselves as far as how they to think, what they eat and even the exercises that they attempt to participate in (Dua et al,2014). The age of the patients will likewise imply that the patients are given solution that can manage them in the most helpful ways and which they can acknowledge all in all. The medication that can be given for this situation is one that can decrease the cruelty of a medication.      Project: Treatment of Hypertension

    Hypertension

Hypertension, also known as high blood disease pressure is a serious condition that ultimately results in damaging of blood vessels leaving an individual prone to heart attack, stroke, and other conditions. Various factors increase the likelihood of developing the disease they include obesity, consumption of a lot of salt, diabetes, smoking, and alcohol intake. Globally, hypertension results in the death of millions of people, but with the right administration, the mortality rates can be reduced. Treatment of hypertension is diverse including lifestyle changes and treatment through drug therapy. Lifestyle approach in the treatment of hypertension involves intake of healthy diets, quitting smoking, physical exercise, and reducing alcohol intake. Considering the complexities that surround the treatment and management of hypertension, this paper seeks to demonstrate that it is crucial to incorporate lifestyle modifications alongside medical treatments in the management of high blood pressure. Scholarly Activity Summary Template & Example Assignment NRS-490.

    Background of the Problem

Hypertension is a worldwide health issue that afflicts many individuals. The causative factors for the condition can be genetic or acquired through poor lifestyle habits. The disease may also present as a co-morbidity to other illnesses such as diabetes and kidney disease or hypertension may lead to the onset of these diseases. Hence, the problem prevails in a vicious cycle pattern making treatment goals difficult to achieve (Wang, Ning, Yang, Lu, Tu, Jin & … Su, 2014). In addition, the advancement of the food processing industry has immensely contributed to the development of hypertension because of the relatively higher amounts of salts and other harmful food additives in the diets. If left unmanaged, hypertension can lead to the development of co-morbidities such as heart disease, diabetes, and stroke. Blood pressure involves exertion of force on the walls of the blood vessels, with the amount of force exerted dependent on the levels of resistance from the blood vessels and the amount of work done by the heart.

    Problem Statement

Estimates issued by the World Health Organization indicate that in the United States alone, over 85 million people are suffering from hypertension. In the low and middle-income countries in the world, urbanization has steadily caused an upsurge in the number diagnosed cases of hypertension due to changes in lifestyle habits. Globally, increased blood pressure has been found to cause over 7 million deaths, a figure which accounts for over 12% of the total world deaths. Overall, both men and women develop high blood pressure, although the prevalence rates are higher for men than for women. Therefore, hypertension is a public health concern that has been found to cause the development of cardiovascular disease and stroke (Khdour, Hallak, Shaeen, Jarab & Shahed 2013).

A lot of research work has been carried out with the intention of deciphering the etiology of the disease and the best treatment approaches to the problem of hypertension. Although the research efforts have centered on the medical perspectives of treatment, exploration of the role of lifestyle modification as a preventive measure for hypertension needs to be carried out (Khdour, Hallak, Shaeen, Jarab & Shahed 2013). Furthermore, lack of adequate awareness about hypertension on the part of the patient substantially influences the onset of hypertension, and the delayed seeking of treatment for the condition. Also, because of an increase in population and the sharp increase of the geriatric demographic age group, the number of people living with unmanaged hypertension in the world has been on the increase; hence this paper advocates for the use of lifestyle modifications in the management of hypertension among adult males. Scholarly Activity Summary Template & Example Assignment NRS-490.

    Purpose of the Change Proposal

Considering the complexities that surround the treatment and management of hypertension, this paper seeks to demonstrate it is crucial to incorporate lifestyle modifications alongside medical treatments in the management of high blood pressure. This is because ideally, lifestyle changes should be considered not only as preventive measures but also as first line treatment options. These adjustments include dietary changes such as consumption of low sodium, high glycemic index foods, and daily intake of 3-5 servings of fruits and vegetables. Physicians and nurses recommend that pre-hypertensive or hypertensive patients engage in physical exercise which includes half an hour of low to moderate intensity aerobic exercise such as swimming, jogging walking or cycling (Nwankwo, Yoon, Burt, & Gu, 2013). In addition, utilizing stress reduction measures significantly assists with blood pressure control. These measures include avoidance of cigarette smoking and alcohol and consumption of unhealthy diets. Scholarly Activity Summary Template & Example Assignment NRS-490

    PICOT Statement

To address the problem of hypertension, the following PICOT question was developed: For male grown-ups between the ages of 40 and 70 with hypertension, and with various co-morbidities, will the adjustment in way of life (drawing in routinely in exercise and eating more beneficial and adjusted suppers), contrasted with patients who utilize prescription to treat/deal with their hypertension, help to control their pulse and diminish the danger of creating cardiovascular illnesses in their recuperation period inside a half year? Literature Search Strategy Employed

For the search of literature review, the Ebscohost data base search was used. Key words used in the search were hypertension, high blood pressure, and hypertension among males. A total of 25 articles related to hypertension were found and another 13 articles discussing high blood pressure and other morbidities were found. Further, no articles related to the topic of hypertension among males were found in the data base. Of the 38 articles found, those that failed to meet search criteria of hypertension and high blood pressure and lifestyle adjustments were eliminated. A total of three articles were left for the literature review.

    Literature Review

As discussed earlier, the problem of hypertension has a high prevalence in the United States and the world. Wang et al. (2013) assert that high blood pressure is the single most significant risk factor in the development of cardiovascular disease.  The authors further state that low awareness lack of treatment and management leads to an increase in the number of deaths and complications from hypertension. Risk factors such as high body mass index lack of physical exercise and consumption of unhealthy diets cause further problems into an already complex issue.

A National Health and Nutrition Survey reveals that early diagnosis and treatment ensures the prevention of complications of hypertension for instance damage of the arteries and blindness (Nwankwo, Yoon, Burt, & Gu, 2013). However, research by suggests that hypertensive patients may fail to develop signs or symptoms in which case treatment is sought for the eventual complications such as blindness. Lifestyle changes in combination with medication may lower high blood pressure and lead to an improved quality of life for the patient. However, lifestyle modifications alone have been found effective as a first line option treatment for hypertension. These changes include the incorporation 30 minutes of physical activity into the patient’s daily routine.

Ideally the goal of physical exercise is to assist the patient with weight loss and a reduction of the BMI (Khdour et al., 2013). Dietary adjustments entail intake of low sodium diets, intake of high fiber diets, which includes fruits and vegetables, and consumption of low-fat dairy products. Lifestyle modifications also constitute the avoidance of stress and its causative factors such as smoking and alcohol consumption. Dietary changes may involve the nurse or the nutritionist assist the patient develop a feeding plan which is known as dietary approaches to stop hypertension diet.

    Applicable Change Model

For patients suffering from hypertension and other chronic diseases, the Chronic Care Model is an effective organizational strategy to caring for hypertensive patients. I this scenario, the model is applicable because it utilizes evidence based research and creates interactions between an educated patient and a proactive team of health care professionals. Moreover, the system is practical and offers support for the patient in management of hypertension (Davy, Bleasel, Liu, Tchan, Ponniah, & Brown, (2015).

    Implementation Plan

Following the six components of the Chronic Care Model, the implementation plan will aim to decrease high blood pressure, manage weight, support self-management and decision making that will impact on the overall well-being of the patient. Self-management is critical because it indicates that the patient is in charge of their own health care situation. Applicable changes will be filtered through the components of the Chronic Care Model and tested against evidence-based research to ascertain that they are effective at achieving the goals of blood pressure reduction and weight reduction (Davy, Bleasel, Liu, Tchan, Ponniah, & Brown, (2015).

    Potential Barriers to Implementation

Potential barriers to the implementation of the plan include patient apathy and non- adherence to the issues such as physical activity and consumption of healthy diets to achieve the treatment goals of lowering blood pressure and weight reduction (Khatib, 2012). Another key barrier from the patient is an attitude of indifference which may result in the patient viewing the plan as inconsequential and non-contributory to the management of their blood pressure.

    Conclusion

As noted, hypertension is among the leading causes of heart attacks, strokes, and other complications. With the right interventions in place, the condition can be managed and treated; hypertension treatment considers two forms of treatment including the use of anti-hypertensive drugs as well as lifestyle modifications. Anti-hypertensive drugs used include diuretics, angiotensin, converting enzymes blockers, beta blockers among others each of the drugs work differently, but all ultimately result in lowering the blood pressure. Therefore, apart from medication, lifestyle changes are also crucial in management and treatment of the disease especially in reducing cardiovascular risks and lowering the blood pressure.
